As a Developmental paediatrician, the patients I see have physical or intellectual or both disabilities. Some are quite severe and have multisystem involvement eg.children with severe cerebral palsy or brain damage. I am often asked to give an intervention level which will determine the extent at which medical services will be provided for the child. The lower the intervention level, the more palliative the care. This means the patient will not be taken to ICU should the need arise. This decision is not easy to make as you dont want to discriminate one child from another and deny the highest level of medical treatment to the those with disabilities. It is also a very difficult concept to explain to parents of these children. You feel like you are playing “God”.
